Request for re-membership

We are delighted to learn that you wish to rejoin the AMQ®. As a professional massage therapist, you deserve simple feedback.

First stage…

In order to simplify your re-membership request, we need to know if you have already had a personal profile with the AMQ®.

The personal profile is a virtual platform, more precisely a secure section in which members has access to their personal AMQ® account.

You have already had a personal profile at the AMQ®

1. First, connect to your personal profile by clicking on the “My Profile” button at the top of the home page of the AMQ® website.
   • Here is the link : https://intranet.amquebec.qc.ca/en
   • If you have forgotten your password, click “Lost Password?” and follow the instructions.
2. Next, update your personal information in your profile.
   • To do this, go to the menu, click on your first name and on “My profile”.
3. Then, send an email to info@amquebec.qc.ca so that we can take out your paper file and start preparing for your re-membership. Simply add the following to your message :
   • Your first name and last name;
   • Your AMQ® licence number;
   • Your phone number;
   • The reason for your message (a re-membership request for which the personal profile already exists);
   • Your certifications, diplomas, certificates, etc., if you have completed studies and/or training since the end of your AMQ® membership;
   • The signed and dated commitment form.
4. We will be happy to contact you as soon as possible to answer your questions and reactivate your file.

You have never had a personal profile at the AMQ®

1. First of all, send an email to info@amquebec.qc.ca so that we can take out your paper file and start preparing for your re-membership. Simply add the following to your message :
   • Your first name and last name;
   • Your AMQ® licence number;
   • Your phone number;
   • The reason for your message (a re-membership request for which the personal profile does not exist).
2. Then, complete the online membership form.

   • Don’t forget to add your certifications, diplomas, certificates, etc., if you have completed studies and/or training since the end of your AMQ® membership.
3. We will be happy to contact you as soon as possible to answer your questions and reactivate your file.
